Most fatalities during re-entry

On 1 February 2003, the space shuttle Columbia was lost with all seven crew during re-entry into Earth´s atmosphere after a successful 15-day mission in orbit. The resulting investigation determined that a piece of insulating foam from the external fuel tank had impacted the leading edge of a wing during launch. This made a hole that allowed heat to build up during re-entry, causing the left wing of the orbiter to break apart, and the rest of the spacecraft to disintegrate. The crew were Rick D Husband, William C McCool, David M Brown, Kalpana Chawla, Michael P Anderson, Laurel B Clark, Ilan Ramon (all USA except Ramon, Israel).
